To set up an initial examination, please contact us via our phone number (734-422-1300) or in person. You may also request an appointment via our book now link and will receive a confirmation if your selected time is available. All initial examinations are performed by a Doctor of Physical Therapy and take roughly 60 to 90 minutes with a plan of care to be determined following completion. Initial examination paperwork will be sent via email or text message. Please arrive 15 minutes prior to scheduled appointment time if choosing not to fill out paperwork beforehand. In addition to paperwork, please also bring a photo ID, insurance card, and physician prescription for therapy.
