To set up an initial examination, please contact us via our phone number (734-422-1300) or in person. All initial examinations are performed by a Doctor of Physical Therapy and take roughly 60 to 90 minutes with a plan of care to be determined following completion. Initial examination paperwork is below in PDF form. Please arrive 15 minutes prior to scheduled appointment time if choosing not to fill out paperwork beforehand. In addition to paperwork, please also bring a photo ID, insurance card, and physician prescription for therapy.
